PHYSICALS AND ONLINE CLEARANCE

All returning and incoming athletes must have completed the Athletic Clearance process before they can participate in any summer and/or school-year sports activities. The Athletic Clearance is valid for one year and must be repeated on an annual basis. Failure to renew the Athletic Clearance prior to summer will result in the student not being able to participate in any summer and/or school-year activities at Antelope Valley High School.

Participation is only allowed after the Athletic Clearance has been approved by Antelope Valley Athletic Department. Since the 2017-2018 school year the Antelope Valley Union High School District will be using an online Athletic Clearance Process. The only papers that need to be turned in are the physical exam certification that is signed by a medical professional and the bus waiver. Step by step instructions for completing the online process with screenshots matching each step are listed in the AVUHSD Athletic Packet. You will need your physical document, insurance information, and student present in order to complete the online process.

The physical exam certification that is signed by a medical professional can be done by your healthcare provider. However, Dr. Massari is currently open and athletes can bring $20 and get a physical there quickly.

Students must be cleared both online AND with the physical form to participate in athletics.

Students can go to their primary physician with the form and also get cleared (during a wellness visit, etc.).

The clearance process is two steps:

A. The online clearance process:

1. Go to https://athleticclearance.com/

2. Use your username and password from the previous year (if not, they will have to retype everything in a new account). If new, follow the form below.

4. Important: Choose the date for "Year" (for example, 2021-2022)

5. Choose Antelope Valley (CIF-SS) for "School"

6. Choose the first sport you are getting cleared for under "Sport"

7. Follow Steps #1-#4

8. Be sure to click on other sports if the athlete plays multiple sports.

B. The physical clearance process:

1. Bring a copy of the "blue card" to your physician:

2. Parent/student need to fill out the student information on the top of the form

3. Have the physician fill out the form in the middle section: sign and date.

* For football the insurance policy and parent initial MUST be signed on the parent authorization section.
