Athletic Clearance Checklist
Completion of both the online clearance and the physical exam is required before participation.
Step A: Online Clearance
- Visit athleticclearance.com.
- Log in with last year's account or create a new profile. Select "Start Clearance Here."
- Choose Year: 2020-21, School: Antelope Valley (CIF-SS), and the sport you are clearing for.
- Complete all four online steps and mark additional sports if you plan to play multiple seasons.
Step B: Physical Clearance
- Print the AVUHSD physical form ("blue card") and fill out the student section.
- Schedule a physical exam with a doctor, such as Dr. Massari, and have the provider complete, sign, and date the form.
- Parents sign the authorization section, including the insurance policy initials required for football.
$20 Physical Exam
Dr. Massari offers sports physicals for $20. Students must complete both the online clearance and the physical exam before they may participate.
